8 – Identifying Good Practice
This topic explains the meaning of ‘good practice’. You will also learn why the investigation team look for examples of good practice.
9 – Identifying Care Delivery Problems
Topic 9 explains the meaning of the term ‘Care Delivery Problem’. You also will learn how to word these problems precisely.
10 – Contributory Factors, Systems and Recommendations
This topic explains the term ‘Wagon-Wheel Analysis’. You will learn about contributory factors and how the team identify missing or failing systems. The topic also explains how the team come up with recommendations and why they do not identify a definite root cause of the care delivery problem.
11 – Report, Debrief and Follow-up
The final topic identifies what the investigation team will put in their final report. It also covers the feedback they will give to staff who were involved in the incident.
